High school students who wish to play a sport MUST complete the PUHSD paperwork, which includes having a physical signed by a physician that is dated after June 1, 2025, and complete the online Athletic Clearance. This needs to be completed before they can participate in sports, (this includes tryouts, practices, weightlifting/training and any summer training/camps.

The online clearance can be attained by either creating an account or updating an existing account at www.homecampus.com. This will be available 5/31/25
Please make sure that you complete all sections online.

You will then need to upload the insurance card. Please do not upload the Physical!!!

The original PUHSD paperwork will need to be returned to the Athletic Office, please don't give it to the coach. No email copies will be accepted.
You are able to select multiple sports online but please limit it to 3. This can always be changed in the future by you, the student or the Athletic Office.

Once all paperwork is received and reviewed you will then receive an email stating that your student is clear to participate in sports. A clearance list will be given to each coach prior to tryouts, summer camps or weight training. If your student is not clear they will not be able to participate.
